• Fill in the forms on-line
• Print out the completed application form  

Duly completed forms should be accompanied with the following documents:

• A copy of your international Passport with a valid stay documentation (visa, permanent residence or work permit)
• A copy of a utility bill or  valid driver’s licence
• Two passport-sized photographs  

The completed application forms and the required documents can be scanned and attached to an email to us at nidahelpdesk@bankphb.com
